Polygenic risk score test ordering page

Your provider has offered the opportunity to obtain a score which assesses your future risk of a heart attack or stroke based on your genetics.

Please enter your contact information and Provider ID (given to you by your Penn physician), and we will mail you a kit. Once you return the kit with a saliva sample, we will process the information and provide results to your provider in 4-5 weeks. Your provider will be able to review this information with you at your next scheduled visit.

Please note that we charge $150 for this testing service, which is a discounted rate offered exclusively to patients of the Penn Medicine Center for Preventive Cardiology and Lipid Management.
